A complete extension application starts with a precise scope. This checklist helps the preparer collect the right information before opening Form 8809 or an electronic service. It uses the current December 2025 form and 2026 IRS guidance, with a fictional application map rather than invented taxpayer details.

Identify the return and deadline being extended

Start with a single payer or filer, reporting year, exact return type and agency filing method. Write down the original filing deadline and the official source that establishes it. Keep the recipient statement date in a separate field. Form 8809 addresses filing information returns; it does not extend furnishing, state reporting or a tax-payment deadline.

The December 2025 Form 8809 instructions cover only the returns listed on line 6 for the current tax year. They direct filers to apply as soon as the need is known, no earlier than January 1 of the year the return is due, and by the applicable filing deadline. A request filed after that deadline cannot receive an extension under this procedure.

Collect the filer's identity and contact information

Obtain the current legal name, tax identification number and complete correspondence address from controlled business records. The instructions require the legal name and TIN to match the IRS registration, accounting for a name change already submitted to the IRS. Do not shorten a long legal name merely to match an internal nickname.

Line 1 also needs a contact who understands the application, telephone number and email. If correspondence should go to a preparer, preserve the client's full legal name and use the care-of arrangement described in the instructions. Line 2 calls for the applicable nine-digit identifier without hyphens. Keep an internal reference in the working checklist instead of copying the full TIN into routine email.

Determine whether the request is automatic

For listed returns other than W-2 and 1099-NEC, an initial timely request generally provides an automatic 30-day extension without a justification. W-2 and 1099-NEC require a nonautomatic paper request, an applicable line 7 criterion and an authorized signature. They have only one possible 30-day extension; no additional extension is allowed.

An additional extension for another eligible return type is also nonautomatic and paper-only. Confirm the initial extension was granted, that the request concerns the same year and forms, and that it will be filed before the first extension expires. Do not check line 5 simply because the company filed an extension in a previous reporting cycle.

Use a preparation worksheet before completing the form

In this fictional example, Alder Lake Services needs extra time for electronically filed Forms 1099-MISC. Its payroll team also has an unrelated W-2 problem. The preparer creates separate work items so the MISC automatic route does not conceal the W-2 hardship review.

The underlying W-2s can be electronic even though their extension request must be on paper. Keep those two method decisions distinct when reviewing line 3 and selecting the application route.

Verify the route and assemble attachments

Publication 1099 for 2026 directs eligible electronic extension requests through IRIS. The December 2025 Form 8809 also lists legacy FIRE options. Check the current IRS service guidance and the applicable year's instructions before using a saved procedure, especially during the system transition. A previously working bookmark is not enough to establish current availability.

For multiple payers, line 4 requests their count and a typed list of names and TINs. That is a count of payers, not individual returns. The form's legacy fill-in FIRE method cannot accept that attachment. Confirm that the selected current route supports the intended application structure. Use separate forms for different underlying filing methods as line 3 requires.

Review the application and preserve what was sent

Have a reviewer compare the completed application with the source worksheet: identity, year context, earliest applicable deadline, return selections, method, attachments and initial versus additional status. For nonautomatic requests, verify the actual qualifying circumstances and authorized signature. Do not select a hardship criterion merely because the team has unfinished work.

Save the final application, required attachments and method-appropriate submission evidence. Record the result or correspondence separately from the act of sending. Continue production of the underlying returns, with owners for remaining data and review tasks. Track recipient furnishing independently, using Form 15397 only where its separate extension procedure applies.

Common questions

Does every Form 8809 need a hardship explanation?

No. Initial automatic extensions for eligible forms do not require justification. W-2, 1099-NEC and additional extension requests require the applicable line 7 criterion and a signature under the current form instructions.

Which filing method belongs on line 3?

The method for the underlying information returns. It is not asking how you send Form 8809. Use a separate Form 8809 for each underlying method when both electronic and paper returns are involved.

Can I prepare a request before January?

You can gather records and plan earlier. The instructions say to file no earlier than January 1 of the year the return is due. Preparation and actual submission should have separate dates in your register.

Does a W-2 extension request go electronically?

No. Current Form 8809 instructions require W-2 extension requests on paper. They are nonautomatic, even when the employer will electronically file the underlying wage statements with SSA.

Can I use Form 8809 for recipient statements?

No. Its scope is agency filing. Review Form 15397 and the applicable return instructions for furnishing extensions, and retain a separate deadline and evidence record for that obligation.
