The practical answer

An initial 1099-MISC extension example uses the payer's identity, the underlying return method and the correct return selection, while leaving additional-extension and hardship fields unused.

Form 8809 is short, but several fields are easy to misread. Follow one fictional application from identity through release, then compare the fields that change for nonautomatic requests. The accompanying field diagram and review sheet make the decisions visible without using real taxpayer data.

Set the facts for the fictional application

This annotated example uses the December 2025 revision of Form 8809. Orchard Lane Management, a fictional payer, needs an initial extension for electronically filed Forms 1099-MISC. It has one legal entity and no earlier extension for this year. The example omits real taxpayer identifiers and addresses; it is a preparation model, not a form to submit.

Before applying this model, verify your own return's current-year filing deadline and eligibility. Form 8809 must be filed by the original due date and no earlier than January 1 of the year the returns are due. The printed form uses recurring month-and-day rules, so consult the applicable year's instructions for actual dates and holiday adjustments.

Lines 1 and 2: show the payer correctly

Line 1 identifies Orchard Lane Management by its full IRS-recognized legal name. The preparer enters the correspondence address, including any suite information, and a knowledgeable contact's name, telephone and email. If an outside preparer receives correspondence, the client remains the named payer with an appropriate care-of address.

Line 2 uses the payer's applicable nine-digit TIN without hyphens. Our worksheet uses a secure identity-record reference instead of printing a pretend number that could be mistaken for valid data. Verify the name and TIN against the IRS registration, including a properly submitted name change. The software provider's identifier does not replace the payer's identifier.

Lines 3 and 4: separate method from volume

Orchard Lane checks electronic on line 3 because it plans to e-file the underlying information returns. This selection does not describe how the extension request itself is delivered. If an organization has underlying returns filed by both methods, the form requires a separate Form 8809 for each method.

Line 4 concerns a request covering multiple payers or filers. Orchard Lane has one payer and submits an individual application, so it does not enter a number there. Its 240 fictional recipient returns do not belong in this field. Where one permitted application covers multiple payers, enter the payer count and provide the required typed list of names and TINs.

Lines 5 through 7: annotate the decisions

For this initial MISC request, line 5 remains unchecked. The applicable grouped selection on line 6 includes Forms 1097, 1098, 1099, 3921, 3922 and W-2G. Form 1099-NEC has its own separate selection and special procedure. The preparer confirms the actual return is MISC rather than assuming every 1099 shares the same treatment.

The form says to check applicable return boxes, not enter the number of returns. Keep recipient volume in the internal work plan where it helps estimate the remaining preparation effort.

Change the example for a nonautomatic request

If the underlying form were W-2 or 1099-NEC, this automatic example would no longer apply. The request must be on paper, meet an applicable line 7 criterion and bear an authorized signature. Only one 30-day extension is available for those forms. An additional request for another eligible form is also nonautomatic and requires line 5, line 7 and the signature.

Use the form's actual criteria, such as qualifying casualty effects or the specified responsible-person circumstances. Document real facts and dates. An ordinary workload backlog does not authorize selecting a criterion that did not happen. Current Form 8809 has numbered lines and a signature area, not a separate hardship section called Part II.

Review, submit and retain the completed version

No signature is required for the initial automatic request in this MISC example. Review the form against the worksheet before release. For current eligible online requests, Publication 1099 for 2026 points to IRIS; the older December 2025 form still prints FIRE options. Verify the current service rather than copying a historical screen sequence.

Retain the application version and submission evidence, then continue preparing the actual returns. Do not attach Form 8809 to those returns as a general practice. The instructions contain a specific exception for paper Form 8027, which requires a copy of the timely filed application. Recipient furnishing remains a separate obligation, outside this illustrated application.

Common questions

Where do I enter the number of recipient forms?

Do not enter that count in line 4 or line 6. Line 4 is for the number of payers or filers in a multiple-payer request, while line 6 selects return types. Keep recipient volume in your internal worksheet.

Is this a completed form I can submit?

No. It is a fictional annotated field guide. Prepare your application using verified legal identity, the applicable current-year deadline and the current official form or supported electronic process.

Does the initial MISC example need a signature?

The current form instructions say no signature is required for an automatic 30-day extension. W-2, 1099-NEC and additional requests have different signature and justification requirements.

Can a preparer's name replace the client on line 1?

No. The instructions permit a care-of correspondence arrangement, but the client's complete legal name remains the payer identity. Confirm that the TIN also belongs to the correct filer.

Should line 5 be checked because we extended last year?

No. It concerns an additional extension for the same year and forms after the initial automatic extension. A prior-year application does not make this year's first request additional.
